2008 registration forms

There are 2 ways to enter:
Please read your entry form carefully; one entry form must be completed for each sport entered. Please print legibly and remember to read and sign the waiver. If you are under 18, a parent or guardian must also sign the waiver.

1. By mail or in person.
   Early entries are encouraged in order to avoid late fees and long lines. Entries will be accepted until 5 p.m. on the published date of your sport's deadline. Athlete packets will be available for pick-up at registration on Friday, June 20, 2008, or at your venue.
   
   
2. Registration day.
   Registration day. Entries will be accepted for onsite sports on Friday, June 20, 2008, from 8 a.m. to 5 p.m. at the State Games of Mississippi Command Central Office, Meridian Community College on 14th Street. Applicable late fees will be added. Athlete packets may be picked up at this time by individual athletes and team coaches.

• If the information is not complete or the waiver is not signed, your entry form will be returned.
  
  
• Confirmation materials will be mailed to all complete entries post marked by June 6, 2008.
  
  
• Team confirmation materials and team roster/waivers will be mailed to the coaches. Deadlines vary by sport. All roster waivers not completed and signed by individuals and parent/guardian will be returned to the coaches. Incomplete roster/waivers may result in individuals not being allowed for participate in State Games.

Participation is limited in some sports. Submission of entry form and fee does not guarantee entry.
